Healthroos provides multidisciplinary stroke rehabilitation across Greater Sydney, combining NDIS physiotherapy, occupational therapy, and exercise physiology — all delivered at home, all coordinated as one team.
For NDIS participants, we work with support coordinators, plan-managers, and your treating team to continue rehab goals without the gaps that can happen after hospital discharge.

How We Support Each Stage of Recovery
Every stroke is different. Here’s what rehab typically looks like across recovery phases.
PHASE 01
Early Recovery (0–3 months post-discharge)
Clinical focus: Regaining functional movement, preventing secondary complications, safe mobility at home.
Example interventions: Bed/chair transfers, early gait retraining, home safety assessment, carer education, upper-limb stimulation, swallowing/communication referrals where needed.
PHASE 02
Consolidation (3–12 months)
Clinical focus: Building strength and endurance, expanding daily independence, community re-engagement.
Example interventions: Progressive strengthening, task-specific practice, cognitive rehabilitation, AT prescriptions, graded community outings, driving assessment referrals.
PHASE 03
Long-term Maintenance (12+ months)
Clinical focus: Preventing decline, optimising function, supporting meaningful participation.
Example interventions: Ongoing exercise programming, secondary prevention, annual FCAs, plan-review reports, transition to community gym or sport.

- Adults discharged from acute or subacute rehab settings

- Families and carers who need practical strategies and education
We coordinate with hospital discharge planners, treating neurologists/rehab physicians, support coordinators, and GPs to ensure rehab continuity.
Stroke Rehabilitation FAQs
Home-based rehab can start as soon as your treating team clears discharge from hospital or inpatient rehab. We respond to referrals within 4 business hours and can often begin assessments within 1–2 weeks.
Yes. We coordinate with treating specialists, hospital discharge planners, and support coordinators to continue rehab goals at home without gaps. We can receive handover notes and share progress reports back with the broader team.
For NDIS participants with stroke listed as a primary or secondary disability, rehabilitation may be funded under Improved Daily Living or Improved Health and Wellbeing budget categories. Your support coordinator can help confirm which budget applies.
Yes. Our multidisciplinary team is experienced with all stages of stroke recovery — from early subacute rehab through to long-term maintenance and community reintegration.
