Does this confirm an appointment?

No. It is an initial capacity enquiry. We will contact you about service fit, current availability and the next step.

Do I need to send a referral or plan?

No. Please do not send plans, reports or detailed clinical files unless our team asks for them after reviewing your enquiry.

Can a coordinator, case manager, family member or provider enquire?

Yes. The form will ask for your contact details and the minimum participant or client details needed to assess capacity.

Can I request a male or female clinician?

You may share a preference. We will consider it alongside clinical fit, location and current availability, but cannot guarantee a particular clinician.
